Allow the administrator to specify a "prefix" code to be prepended to dial strings for trunk selection

Description
Allow the admin to specify a prefix that will be applied to outbound calls - allowing the module to select a trunk by prefix code.
Alternately - we could provide a drop down box allowing the admin to select a specific trunk.

I have sorta of begun working on this. I think Im going to use the "select trunk from a drop down box" method.

This upgrade is coming along slowly, but it is coming along. At this time, the logic to build calls files for both inside and outside calls is in place - which is needed to be able to specify a trunk to dail out on is almost code complete.
Some testing of the outbound outside call lagic still needs to be done.
